To what extent Hreflang content should be changed?

To what extent am I suppose to change the content on my different language page/domain when using Hreflang property?

If I publish the very same content on . com & . ru without changing anything will it be duplicate as I haven't change a character even although I marked hreflang.

Can I target synonym keyword in different Hreflang pages or change meta tags as well?

What all changes shall I make?
