How do I make Google remove those pages with for good(not just 90 days)?

When I published my page to google, I had no <meta name="robots" content="nofollow,noindex"/> in it.

Now I do, but it does not leave the index.

How do I make Google remove those pages with <meta name="robots" content="nofollow,noindex"/> for good(not just 90 days)?
