Desperate for SEO Guidance - Small Counseling Center

I'm the owner of a counseling center with 5 clinicians. I was in private practice myself when I decided to take the leap but now it seems I'm spinning my wheels when it comes to online referrals as we receive very little (2-3 a month) but I know this is a huge market we're missing out on.

Long story short, I hired a company who ended up over-promising and under-delivering and I paid around 5k altogether (I know this is cheap) for site and SEO. It was a friend and I feel he did his best but he became too busy with other projects to make it a priority. Anyway we had to redo the website because of technical problems at a completely different domain which meant we had to start over. We're a month into the new website but I feel like we're not ranking because of backend problems we're getting knocked for that I don't have an understanding.

We have about 12 backlinks in a month so I'm pleased with this and these are all organic. I'm constantly firing out keyword rich blog-posts (although I'd like insight on where to go to find the best way to write them) and doing outreach to be sure they're making it on local affiliate sites. Is there anything that I should be doing more of to make sure I've done everything on my end?

How do you know a company is reliable and what is a reasonable fee to pay someone who does web development (not content creation) so your site is SEO optimized (meta-descriptions, alt attributes, title lengths, slow loading times) while ranking keywords that aren't going to screw me by subscribing me to shitty directories with poor backlinks? I'm willing to learn and I'm a quick study but the same person who helped me before said google is changing their algorithm. When this happens, is all my work for going to be for none?
