Avoiding Duplicate Content Penalty on Cloned Local Sites?

I’m working with a company that is developing a system to spin up similar medical sites for clinics that would include the same content.

The sites will be the same structure, same content, different template and in different cities.

I’m curious about duplicate content issues and if it matters in different cities.
