When a seo company charges per month to do on-page seo, what are they doing each month?

I'm a graphic designer and was quoted $900 a month for on-page seo, recommendation is for 12 months, no other seo work was suggested.

I understand it’s a lot of work to set it up, but after that, what work would they be doing?

The seo company suggested a 10 keyword campaign targeting my city, and mentioned 'keyword research' is what they do ongoing.

But how much work is involved in that each month and how much do keyword searches change or alter over a few months to warrant this research?

I have no issue with what they charge, but it seems like a strange way to structure the payments. Wouldn’t it make more sense to charge more $$ at the start, then less each month to monitor and tweak it?

Not a complaint, just curious about how this aspect of it works...
