How do you actually test page speed consistently?

Here's my issue:

Tools like Google Pagespeed Insights always seem to give ridiculously low numbers.

For example, even testing an SEO expert's site who knows what they're doing, you get awful pagespeed numbers with these tools.

I just put in Nathan Gotch's site and got a score of 36. That's in the red.

How do you actually test pagespeed? It seems like the number is always just absurdly low with these tools and it doesn't make any sense because when you actually load the page, it does just fine.
