Optimizing images and page rank

From a purely theoretical standpoint (or if anyone's had any experience with this), could images on a page help a page rank better if say, the image URL contained prime keywords or was super relevant, but the image lacked an alt tag? Or would the image help the page if the image title was super optimized, vs. an SEO not adding a title for the image whatsoever? Basically, what are the biggest factors that play in to an image on-page and which are less impactful?
